POSITION TYPE: Case Manager – Full Time Non-Exempt

DESCRIPTION: The General Case Manager is responsible for working with homeless individuals to develop a personalized case management plan that addresses barriers to returning to housing. This includes benefits and health care advocacy, budgeting assistance, short- and long-term planning for housing and employment search, education advocacy and connection to other services needed.

REPORTS TO: Case Management Manager of Expansion Programs

SALARY: $26.00 per hour

Duties and Responsibilities:

Ø Assess the service needs of prospective clients and develop individualized service plans with clients.

Ø With Housing Navigator and Housing Case Manager, supports housing counseling and coordinates moves to permanent housing.

Ø Maintain Client Records

o Maintain accurate documentation of service objectives and outcomes as well as other services in accordance with Federal, State, County and Ascencia guidelines;

o Maintain client related data systems, including case notes and HMIS entries;

o Prepare case management related reports including but not limited to: outcomes, successes, etc;

Ø Client Follow-up

o Continually monitor and evaluate each client’s progression through their housing case management plan, and develop corrective action revisions to the plan as needed;

Ø Research and update client resources

o Attend program-related community, coalition and committee meetings as assigned;

o Maintain up-to-date records of providers and resources

Ø Case Conferencing

o Ensure effective service delivery by coordinating work with Ascencia and other service providers.

Ø Housing Counseling

o Work with clients to develop individualized housing plans;

o Adhere to strict boundaries and professional ethics in the care of others;

o Assist clients with housing applications, completes supportive and subsidized housing paperwork, survey rental market for affordable housing, and advocates for clients with prospective landlords.

Qualifications and Requirements for this Position:

§ 4 year degree in human services or comparable combination of education/work related experience required;

§ Experience in social service setting with working knowledge of case management systems and planning techniques;

§ Knowledge of homeless and housing related resources, housing rights issues, the Coordinated Entry System, and the Continuum of Care for homeless persons preferred;

§ Experience working with homeless individuals and families preferred;

§ Must be proficient in the following computer applications--Microsoft Word and Excel; must be able to learn and use client database for recordkeeping. HMIS experience preferred.

§ Must have own transportation to conduct job related travel and must be able to drive agency vehicles.

§ Must possess a valid driver license and meet insurance carrier’s requirements for approval to drive.

§ Employees must carry active and adequate auto insurance corresponding to regulatory requirements if driving any vehicle not owned or leased by Ascencia.

Physical Working Conditions and Office Setting Description

· Some travel required to field locations to facilitate execution of responsibilities. Must maintain a current driver’s license and valid auto insurance with a minimum of state required levels

· Requires prolonged sitting, standing, frequent bending, stooping and stretching

· Some lifting of up to 25 pounds may be required

· Frequent and prolonged typing, as well as operation of a computer. Use of keyboards and phones required
